HCFA Insurance Forms
Effective in 1992, all insurance claim forms submitted to Medicare must be submitted on form HCFA-1500, which is designed for processing on high speed OCR scanning equipment in red "drop-out" ink on special bond paper in strict compliance with government printing specifications. The HCFA-1500 is available with or without a bar code, which may be required by some agencies.
HCFA Jumbo Envelopes
The need to mail the HCFA-1500 claim forms unfolded becomes increasingly important because jams can occur when the forms are folded, which delays processing your claim. Jumbo (9 1/2" x 12 1/2") self sealing window envelopes hold up to 50 unfolded forms and are available with the window on either the right or left side.
